Lots of old Amusement Parks have been long forgotten. Here is link to some photo presentations with the history.

Great listing, I'd also add FAIRYLAND PARK to the list. This was on 39th and Harlem in Lyons, open until roughly 1977. Not much online regarding this park, but I did find a couple of links:

Six Flags-Great America killed most of them off, especially Old Chicago. But, also, smaller portable carnivals, now seen in parking lots, took the low end.

Kiddieland in Melrose Park would still be around, if not for the family squabbles of the owners.
